What the form covers
- Current EPO issue and expiry dates
- Changes in production capacity, raw materials or products
- Waivers or exemptions from EPO conditions
- New process units or storage areas, and whether RC approved them
- New chemicals added to the process
- Requests to modify EPO conditions
- EPO conditions not being met, with the reason and the action taken
- Changes in emissions, waste or wastewater, or any new pollutant, waste or wastewater stream

Documents the form asks for
- The current EPO with its conditions
- RC approval letters, where the form asks about approved changes or waivers
- As applicable: the Air Emission Inventory Report, the Cooling Tower Drift Loss Test Report, the Waste Reduction Plan and any other document requested in the current EPO
